Home Health and Hospice of Person County provides skilled services to those who meet admission criteria. Home health services include nursing, physical therapy, speech therapy, occupational therapy, home health aide, and medical social work. Hospice services include nursing, medical social work, home health aide, physical therapy, occupational therapy, speech therapy, chaplain services, volunteer services, medications related to terminal diagnosis, durable medical equipment, respite or inpatient care, and bereavement care.

Home Health

Home health provides services that make it possible for patients to stay in their home when they have a medical condition that can be managed at home. These conditions can include recovery from illness or surgery, chronic disease care, care for physical disabilities, and rehabilitation treatment.

Home Health Fees

Home health services are covered by Medicare, Medicaid, contracted private insurance, VA benefits, contracted Medicare Advantage plans and self pay.

Admission Criteria

To qualify for home health several criteria must be met:

  • patient must be homebound
  • patient must be under the care of a physician
  • the care must be provided in the patient’s place of residence (home, assisted living facility, group home, family care home, rest home)
  • the services must be skilled and needed on an intermittent basis

Service Area

Services are provided in Person County and the bordering counties with the exception of Virginia.

Home Health Services Available

The primary home health services are skilled nursing, physical therapy, and speech therapy. Secondary services are occupational therapy, medical social work, and home health aide. To qualify for the secondary services, the patient must be in need of a primary service.

Home Health and Hospice of Person County is Medicare/Medicaid certified, licensed by the state of North Carolina and accredited by the Accreditation Commission for Health Care, Inc.

Referrals for Home Health Care

Most referrals come from either a hospital, physician, or rehabilitation or skilled nursing facility. The patient should always be offered a choice of agencies.

Hospice

Hospice provides care for terminal patients and support for their families and caregivers. Hospice meets the physical, psychosocial, emotional, and spiritual needs of the patient. Hospice care focuses on comfort care and pain management in order to allow patients quality of care during the end of life. Hospice believes the quality of life is as important as the length of life.

Hospice Fees

Hospice services are covered by Medicare, Medicaid, contracted private insurance, VA benefits, and donations. No one is denied care due to inability to pay.

Admission Criteria

To qualify for Hospice services:

  • patient must be diagnosed with a life limiting illness of 6 months or less
  • patient and family must be ready for comfort care instead of cure
  • patient must be under the care of a physician
  • patient’s home must be safe for provision of care.

Service Area

Services are provided in Person County and the bordering counties with the exception of Virginia. Care can be provided in the patient’s home, assisted living facility, rest home, family care home, or skilled nursing facility.

Hospice Services Available

Hospice provides nursing care, hospice aide services, a social worker for counseling and support, medication related to the terminal diagnosis, medical supplies and equipment, a chaplain if desired for spiritual support, volunteers for relief for the caregiver, hospital care if needed, and bereavement care after the death of the patient.

Referrals for Hospice Care

Most referrals are made by a physician or a hospital discharge planner.
